Long term results of valve-sparing aortic root reconstruction

چکیده
Methods Between June 1996 and November 2012, 21 patients who underwent aortic valve sparing for aortic root aneurysm were retrospectively reviewed. Mean age was 45 ± 16 years. 9 patients had Marfan syndrome and one patient had Takayasu arteritis. All patients had tricuspid aortic valve except one patient with bicuspid valve, and 15 patients showed moderate or severe degree of aortic regurgitation. 15 patients were diagnosed with annuloaortic ectasia, 2 with ascending aortic aneurysm, 2 with aortic dissection and 2 with sinus valsalva aneurysm. Reimplantation of aortic root was performed in 13 patients and remodeling techniques in 8. Mean follow up duration was 69.4 months.

Aortic root reconstruction in two patients with chronic aortic dissection by aortic valve-sparing procedures using a new aortic root conduit with the sinuses of valsalva (De Paulis Valsalva graft).
The durability of the aortic valve after aortic root reconstruction by an aortic valve-sparing procedure is of particular concern because of the absence of the sinuses of Valsalva in the David type-I reimplantation method. Various improvements have been made to the David-I method. In particular, a new aortic root conduit with the sinuses of Valsalva was developed recently and is expected to imp...
متن کاملDifferent techniques for aortic valve repair and the associated root reconstruction – prospective long-term follow-up of the first 100 patients
INTRODUCTION The advantages of aortic valve and aortic root reconstructive surgery include the provision of natural postoperative valve hemodynamics and the avoidance of prosthetic valve-related complications. A systematic approach based on functional classification of aortic regurgitation allows standardization and reproducibility. Its potential applicability, however, is limited by the relati...
متن کاملSurgical Management of Aortic Root Dilatation with Advanced Aortic Regurgitation: Bentall Operation versus Valve-sparing Procedure
BACKGROUND Although the aortic valve-sparing procedure has gained popularity in recent years, it still remains challenging in patients with advanced aortic regurgitation (AR). We compared the long-term outcomes of the aortic valve-sparing procedure with the Bentall operation in patients with advanced aortic regurgitation secondary to aortic root dilatation. MATERIALS AND METHODS A retrospecti...

T he list of long-term complications after surgical intervention for acute type A aortic dissection includes recurrent dissection, aneurysm formation, and aortic valve regurgitation. Ten years after the initial operation for acute type A dissection, the reoperation rate for these kinds of long-term complications is 13% to 30% in the literature.1 The diseased aortic root in these patients is com...
متن کاملSurgery of the aortic root: should we go for the valve-sparing root reconstruction or the composite graft-valve replacement is still the first choice of treatment for these patients?
OBJECTIVE To compare the results of the root reconstruction with the aortic valve-sparing operation versus composite graft-valve replacement. METHODS From January 2002 to October 2013, 324 patients underwent aortic root reconstruction. They were 263 composite graft-valve replacement and 61 aortic valve-sparing operation (43 reimplantation and 18 remodeling). Twenty-six percent of the patients...
متن کاملEarly results of a novel technique for ring-reinforced aortic valve and root restoration.
OBJECTIVES Surgery for aortic root aneurysm without valve stenosis is increasingly being transformed from the Bentall procedure to valve-sparing aortic root remodelling or reimplantation. In this report, a new repair option is explored, with full functional 'restoration' of the aortic root complex using a geometric annuloplasty ring, leaflet repair, and sinus/ascending aortic replacement with a...
